How to add a coverage plan rate?

Go to Settings screen.
Select the Billing button and then press the Insurance Companies button.
The following screen will be displayed:

Select the Insurance Company you wish to add the rates group.
Then select the coverage.

Select the rates group to apply to coverage in the Billing Schedule field and press the Save button.
