Description

Screen used to document the details of past deliveries, including abortions.


Access 

  • Through the Side Bar section: To access the Pregnancy in the Progress Note screen, click on the Delivery History located on the Side Bar. If the Delivery History button is not visible, click on the Settings button on the Progress Note toolbar. Choose the Side Bar Settings option. Then, drag and drop the Delivery History item from the right side of the screen into the items window on the lower left corner of any Side Bar category. Click the Save button to apply the changes. The Side Bar will update, and the Delivery History button will be displayed under the chosen category. 
  • Through the Search section: Click on the Search button located on the left side of the screen, then select the Delivery History button. 
  • Through the Patient Dashboard section: In the Patient Dashboard section, click on the OB Summary link, located in the Clinical Evaluation section. Or click on the More button and then select the Open Full Page. Then click on the Deliveries button located at the top of screen. 


Sections

Button
  • Add: Allows to record information about past pregnancies. When this button is pressed, the following options will be displayed:
    • DOB: Date of Birth (If applicable).
    • Type of Pregnancy: ("Single", "Spontaneous Abortion", "Induced Abortion", "Ectopic", "Twins", "Triplets", "Multiple", and "Dead")
    • Weeks and Days:  Weeks old when the patient gave birth or had the abortion.
    • Place: Place where pregnancy or abortion occurred.
    • Delivery Type: ("Cesarean Section, Vaginal): Method that gave birth or had the abortion (if applicable).
    • Anesthesia: (“Spinal”, “Epidural”, “General”): Anesthesia administered to you during delivery.
    • Notes: Notes relating to pregnancy or abortion.
    • Pre-term (true / false): Select if the pregnancy was a pre-term one.
  • Edit: Allows to edit a selected pregnancy or abortion. 
  • Deactivate: Allows to deactivate a selected pregnancy or abortion.
  • Cancel: Allows to cancel any changes made after pressing the “Add” button.
  • Save: Save the changes on the screen.
  • Print: Prints pregnancy history report.
  • Previous: Close screen.

Delivery History: Allows you to filter the “Delivery History” table by the following options: “Active”, “Inactive” and “All”.

Delivery Child Details: The detail of the children during the pregnancy selected at the top is broken down.

  • Add Child: Allows you to add the information of date of birth, gender, height, weight, type of pregnancy, note and diagnosis of the baby born during the pregnancy.
