• Go to the Settings screen. 
  • Select the Clinical button and then press the Referral Templates button.  
  • Press the Add button.
  • Write the Template name and select the provider you want the referral to be displayed. (For example purposes we did not select provider in order for Template to be displayed for all providers)


  • Select the following options: Attach Full Record:, Use e-Referral and Print On Save if you wish to be selected or not.


  • Select the preferred provider you want to send referral.
    •  Note: If specialty is selected and then provider by Default in the report will only be displayed the specialty. If you want the provider name to be displayed in report when creating Template do not select specialty.
  • Write the Reason. 
  • Write the Conclusion.


  • Press the Save button.
