Denominator

13 years old or older  patients attended with at least one closed progress note during the evaluation period. 

Numerator

Patients in denominator that have a Smoking Status registered at least once during the evaluation period. (If patient does not smoke choose the Never Smoker option). 

What to do in EHRez?

Choose the Smoking Status to all smoker patients and the Never smoker option to patients that do not smoke in the Smoking Status screen. The 80% to al patients 13 years or older with the Smoking Status field registered in progress note.
