Important: All documentation submitted over the measures of MU and CQM is for 4510 version and next versions.

Normally a EHRez trainer, or from the Regional Center of Extension of CMS, will help in applying for adoption of incentives of the Electronic Medical Record. Please contact our call center at (787) 625-0440 to coordinate your appointment. We recommend reviewing the following points to help you understand the process.

  • There are two measures you need to comply as a provider: Meaningful Use (MU) and Clinical Quality Measures (CQM).

  • The MU measures are divided in two: the core or totally required and the menu or the ones you choose between several to reach a required amount for by stage. The CQM are similar to the menu that you choose the most convenient to comply with an required amount by stage.

  • To meet with the MU measures there is a minimum required percentage in some cases and in others only is to attest Yes or No. The CQM measures only requires having a percentage higher than 0 to meet the goal.

  • Most of the measures look at the at the attended patients with at least one closed progress note, in the measurement/evaluation period, but can measure progress notes or individual's ones like prescriptions or individual lab orders.

  • The measures to attest Yes or No, typically asks for the record functionality or if the practice was carried out.

  • The measurements periods are for 90 days. There are not by quarters or end of month, they can be adjusted to achieve the highest percentage for you or your practice.

  • The measures are by doctors and the dates/evaluation periods can be different between providers of the same clinic.

  • Some measures may not apply to you depending on the activity of your office. Please read the exclusions.

  • Some measures have alternate measurements, that can be more convenient instead of the originals.

  • Each measure has a detailed screen, press double click or press the loop, where all the patients are listed or elements in the denominators, numerators and exactly those that are pending / or for those failing.
