There are several options for adding a consent form. 


Option #1 

  • In the main menu, press the “Patients” button. 


  • The following screen will be displayed:


  • Select the patient to whom you want to add a consent form.
  • Once selected, press the “Documents” button, located at the top of the screen. 


  • The following screen will be displayed:


  • Then press the “Consent Forms” button, located at the top of the screen. 

 

  • The screen with all the consent forms registered in the system will be displayed.

Note: To add a “Template” of consent forms, you must go to “Settings / Office / Template”, select the main category under “General” and the category under “Consent Form”.

  • Select the consent in the table on the left.
  • Then, press the “Select” button. 


  • The following screen will be displayed:


  • Press the “Replace Keywords and Close” button.

 IMPORTANT NOTE: If the document has a [Signature Keyword] registered, remember that you must have a Signature Pad installed.

  • When the template includes a keyword for the patient's signature, a screen will be prompted to collect the patient's signature.
  • Once the patient has successfully signed, please click the 'OK' button.
  • In this example, the provider's signature will be displayed automatically.

Note: To automatically display the provider's signature, your signature must be registered in the system. Additionally, the provider attending the patient must be indicated or selected in the Default Provider field on the Patient Details screen. If not, the signature of the currently logged-in user will be displayed instead.

  • Once the system has completed the replacement of the keywords, the document will be displayed on the screen.

  

  • The following confirmation message will be displayed:


  • Click the "Yes" button to save the changes.
  • To cancel the process, click on the 'No' button.
  • The following message will be displayed:


  • Press the “Yes” button to indicate that the consent is an advance directive.
  • The document will appear on the right side of the screen.


Option #2

  • Select the "QuickLinks" option, situated at the top of the display.
  • Choose the "Patients" option.


  • The following screen will be displayed:


  • Select the patient to whom you want to add a consent form.
  • Once selected, press the “Documents” button, located at the top of the screen. 


  • The following screen will be displayed:


  • Then press the “Consent Forms” button, located at the top of the screen. 

 

  • The screen with all the consent forms registered in the system will be displayed.

Note: To add a “Template” of consent forms, you must go to “Settings / Office / Template”, select the main category under “General” and the category under “Consent Form”.

  • Select the consent in the table on the left.
  • Then, press the “Select” button. 


  • The following screen will be displayed:
