Screen where the patient's diagnoses are recorded.


Access 

  • Through the Side Bar section: To access the Problem List in the Progress Note screen, click the Problem List button on the Side Bar. If the Problem List button is missing, click the Settings button on the Progress Note toolbar. Choose Side Bar Settings, then drag and drop the Problem List from the available items on the right into a category on the lower left of the Side Bar. Save your changes, and the Side Bar will update to include the Problem List button under the chosen category.
  • Through the Search section: Click on the Search button located on the left side of the screen, then select the Problem List button.
  • Through the Patient Dashboard section: To access the Problem List, click on the link in the Patient Dashboard, or click on the Add button and choose the Open Full Page option.
  • Through the Clinical Evaluation: In the Advanced Progress Note screen, click on the Problem List link, located in the Clinical Evaluation section. Or click on the Add button, located in the Clinical Evaluation section.


Sections


Buttons

  • Print: It allows to print the report of the patient's diagnoses.
  • Add Dx to Encounter: Allows selected diagnoses to be added to the encounter, to be displayed on the “Close Encounter” screen.
  • Review Selected: If the patient has registered diagnoses, they are selected in the first column and this button is pressed, which indicates that the provider verified the selected diagnosis. Performing this action raises the numerator of the Meaningful Use Measure - MUC003 (Stage 1) test.
  • No known Problems: If the patient has no diagnosis recorded. When pressed, it will indicate that the patient has no problem, this action raises the numerator of the Meaningful Use Measure - MUC003 (Stage 1) test.
  • Add: When pressed, the “Select Code” screen will be displayed, so that a diagnosis or several diagnoses can be added. The diagnoses can be searched by ICD-10 or by “Snomed” code.


  • Edit: When you have selected a diagnosis in the table and press this button, the “Edit Problem List” screen will be displayed, to make any changes to the diagnosis.


  • Previous: Returns to the previous screen. 
  • Filters [Status]: Toggle between Active, Inactive, Resolved or All diagnosis.

  • Right Click for the following options:

          

  • Set SNOMED: The SNOMED list associated to the selected problem will be displayed. From the list, the provider will be able to select the corresponding ICD10 (Only applies to ICD10 codes).
  • Edit / Modify: Edit the selected diagnosis.
  • Open Order Set: Opens the Order Set for the selected diagnosis, if configured.
  • Display of “Order Set” in the “Problem List” section of the “Advanced Progress Note” screen:
  • Doble click: Double-clicking on the diagnosis will display the “Order Set” associated with the diagnosis.
  • Last Update by: It will display the user, who last updated the diagnosis.
  • Last Update Date: It will display the date and time, which the diagnosis was last updated.
  • Show Code Mappings: Displays all ICD-9, ICD-10 and SNOMED options for the selected diagnosis.
  • More Information
    • Medline Plus Connect: Opens in your web browser to display information pertinent to the chosen diagnosis.
    • Education Handout: Access the educational materials that have been configured for the selected diagnosis. These can be found under: Settings > Clinical > Education Handouts > Edit Mapping.
  • Convert to ICD10/ICD9: It allows converting ICD9 codes to ICD10 and vice versa.
  • Convert Selected to ICD10: Convert the selected diagnosis to ICD10.
  • Display Grid: Display the diagnosis associated with the patient record, with options to toggle between Active, Inactive, Resolved, or All statuses.
    • : Displays the “Dx History” screen. In this screen the history of the patient's diagnoses can be displayed.


Edit Screen Details


  • Status: [Active. Inactive, Resolved]
  • Date Changed: Date on which any change to the diagnosis was made.
  • Date Added: Recording date, the date the diagnosis was added.
  • Relation with: Determine if it is related to another diagnosis on the Problem List.
  • Search (SNOMED): Section where the list of “Snomed” is searched, associated to the associated diagnoses.
  • Problem Notes: 
  • Snomed Selection: This screen will be displayed, when adding a new diagnosis or code, to select the best Snomed code that applies to it.
