Note: The information displayed on the screen may vary per office or center, depending on the supplier's needs. 

  • Go to the Advanced Progress Note screen.
  • Press the Custom Evaluation button, located in the Side Bar section. 


  • Or, click on the Search button located at the top left of the screen and select the Custom Evaluation option.
  • The following screen will be displayed:


  • In order for the selected information to be displayed in the Clinical Evaluation section, the provider must first configure it in Settings/ Clinical Evaluation Template Settings.
  • Once this process has been completed, proceed to perform the specialized evaluation.
  • Select the options corresponding to the evaluation.


  • If you wish to add a new item, press the right mouse button and select the Add Item option.


  • The Option Editor screen will be displayed.
  • Fill in all the corresponding fields and then press the Save button.


  • Once you have completed documenting the specialized assessment, press the Save button. 


  • It will automatically be displayed in the Clinical Evaluation section.
