There are several options for adding a patient assessment note.

 

Option #1

  • Go to the Advanced Progress Note screen.
  • Click on the Evaluation Notes link, located in the Patient Dashboard section.


  • Or, click on the Add button, located in the Evaluation Notes section of the Patient Dashboard.


  • The following screen will be displayed:


  • Press the Add button, located at the top of the screen.


  • The section will be enabled for the composition of the patient's evaluation note.


  • Once you have completed the evaluation score, press the Save button.


  • In the same way, you will be able to make an evaluation note, through phrases.
  • Click on the Phrases button.


  • The following screen will be displayed:


  • Select the sentence you want to add to the patient's evaluation note.


  • Once selected, press the Apply button, located at the bottom of the sentences.


  • It will automatically go to the notes field.


  • Press the Save button, located at the top of the screen, to save the patient evaluation note.


  • Automatically, the evaluation grade will be displayed in the progress note, if configured in the Progress Template. 


  • The evaluation notes can also be viewed and added in the Tasks screen.
  • Similarly, the evaluation notes can be added and viewed in the Patient Dashboard section, in the progress note.


Opción #2

  • Go to the Advanced Progress Note screen.
  • Press the Evaluation Notes button, located in the Side Bar section.


  • Or, click on the Search button located at the top left of the screen and select the Evaluation Notes button.
  • The following screen will be displayed:


  • Press the Add button, located at the top of the screen.


  • The section will be enabled for the composition of the patient's evaluation note.


  • Once you have completed the evaluation score, press the Save button.


  • In the same way, you will be able to make an evaluation note, through phrases.
  • Click on the Phrases button.


  • The following screen will be displayed:


  • Select the sentence you want to add to the patient's evaluation note.


  • Once selected, press the Apply button, located at the bottom of the sentences.


  • It will automatically go to the notes field.


  • Press the Save button, located at the top of the screen, to save the patient evaluation note.


  • Automatically, the evaluation grade will be displayed in the progress note, if configured in the Progress Template.

Option #3 

  • Go to the Advanced Progress Note screen.
  • Access the patient's clinical task.
  • If the patient has any pending clinical task, press the Active Task button, located in the Side Bar section.


  • Or, press the Search button, located at the top left of the screen and select the Active Task button.
  • The following screen will be displayed:


  • Press the Add button, located in the Evaluation Notes section.


  • The following screen will be displayed:


  • Write the patient's evaluation note.


  • Once you have completed the patient evaluation note, press the Save button located at the top of the screen.


  • The evaluation grade is automatically saved.


  • Likewise, it will be saved in the patient's progress note.
