There are several options to configure the questionnaire when using the self-registration. 


Option #1

  • Click on the QuickLinks text, located at the top of the screen. 

  • Select the Settings option.

  • The following screen will be displayed: 


  • Click on the Clinical button and then select the Clinical Task Templates option. 
  • The following screen will be displayed: 


  • Press the Add Questions button, located at the top of the screen.


  • The following screen will be displayed:


  • Press the right mouse button and select the Add option.


  • The section for the configuration of the question will be enabled. It can be configured in Spanish and English.


  • Press the Save button to save the configuration.


  • The configuration can be made per office. Select it in the Offices field.


  • Press the right mouse button and select the Add option.


  • The section for the configuration of the question will be enabled. It can be configured in Spanish and English. 


  • Press the Save button to save the configuration.


Option #2
  • Press the Settings button in the main menu. 


  • Se desplegará la siguiente pantalla: 


  • The following screen will be displayed:
