There are several options to add a diagnosis to the patient.


Option #1


Recommended option, in order to increase the Meaningful Use measurements.

  • Go to the Advanced Progress Note screen.
  • Click on the Problem List link in the Patient Dashboard section.


  • The following screen will be displayed:


  • Press the Add button, located at the top of the screen.


  • The Select Code screen will be displayed:


  • Select the diagnosis(s).
  • Double-click on the selected diagnosis.


  • Or, select the diagnosis and press the Save button, located at the top of the screen.
    • Note: It is recommended that the provider configure the most frequent codes to appear in the Multiple Code Selection screen, which are configured in the Settings / Clinical / Frequent Codes screen.
  • The Snomed Selection screen will be automatically displayed, if configured in the Settings / General / General screen.
  • Select the appropriate Snomed code and then click on the Save button. Or, click on the Snomed code.
  • Once you have returned to the Select Code screen and have added the diagnosis(s), press the Save button.
  • The following confirmation message will be displayed:

                   

  • Press Yes if you want the selected code to be added to the claim.
  • Press No to cancel the action.
  • It will automatically display in the Clinical Evaluation section if the selected Template has the Problem List option.

  • Likewise, it will go to the Assessment and Plan section, under the Todays Diagnosis category.

  • When you return to the Problem List screen, the diagnosis will already be recorded.


Option #2

  • Go to the Advanced Progress Note screen.
  • Click on the More button, located next to the Problem List link and select the Open Full Page option.


  • Or, press the right mouse button and select the Open Full Page option.


  • The following screen will be displayed:


  • Press the Add button, located at the top of the screen.


  • The Select Code screen will be displayed:


  • Select the diagnosis(s).
  • Double-click on the selected diagnosis.


  • Or, select the diagnosis and press the Save button, located at the top of the screen.
    • Note: It is recommended that the provider configure the most frequent codes to appear in the Multiple Code Selection screen, which are configured in the Settings / Clinical / Frequent Codes screen.
  • The Snomed Selection screen will be automatically displayed, if configured in the Settings / General / General screen.
  • Select the appropriate Snomed code and then click on the Save button. Or, click on the Snomed code.
  • Once you have returned to the Select Code screen and have added the diagnosis(s), press the Save button.
  • The following confirmation message will be displayed:

                   

  • Press Yes if you want the selected code to be added to the claim.
  • Press No to cancel the action.
  • It will automatically display in the Clinical Evaluation section if the selected Template has the Problem List option.

Option #3

  • Go to the Advanced Progress Note screen.
  • Press the More button, located next to the Problem List link, in the Patient Dashboard section and select the Add Problem List option.


  • Or, right-click on that section and select the Add Problem List option.


  • The Select Code screen will be displayed:


  • Select the diagnosis(s).
  • Double-click on the selected diagnosis.


  • Or, select the diagnosis and press the Save button, located at the top of the screen.
    • Note: It is recommended that the provider configure the most frequent codes to appear in the Multiple Code Selection screen, which are configured in the Settings / Clinical / Frequent Codes screen.
  • The Snomed Selection screen will be automatically displayed, if configured in the Settings / General / General screen.
  • Select the appropriate Snomed code and then click on the Save button. Or, click on the Snomed code.
  • Once you have returned to the Select Code screen and have added the diagnosis(s), press the Save button.

Option #4

  • Go to the Advanced Progress Note screen.
  • Press the Add Problem List button, located in the Side Bar section.


  • Or, press the Search button located at the top left of the screen and select the Add Problem List button.
  • The Select Code screen will be displayed.


  • Select the diagnosis(s).
  • Double-click on the selected diagnosis.


  • Or, select the diagnosis and press the Save button, located at the top of the screen.
    • Note: It is recommended that the provider configure the most frequent codes to appear in the Multiple Code Selection screen, which are configured in the Settings / Clinical / Frequent Codes screen.
  • The Snomed Selection screen will be automatically displayed, if configured in the Settings / General / General screen.
  • Select the appropriate Snomed code and then click on the Save button. Or, click on the Snomed code.
  • Once you have returned to the Select Code screen and have added the diagnosis(s), press the Save button.

Option #5

  • Go to the Advanced Progress Note screen.
  • Click on the Problem List link, located in the Clinical Evaluation section, or click on the Add button, located at the end of the Clinical Evaluation section. Or, click on the Add button, located at the end of the Clinical Evaluation section (It must be set in the Progress Notes Templates Settings screen).


  • The following screen will be displayed:


  • Press the Add button, located at the top of the screen.


  • The Select Code screen will be displayed:


  • Select the diagnosis(s).
  • Double-click on the selected diagnosis.


  • Or, select the diagnosis and press the Save button, located at the top of the screen.
    • Note: It is recommended that the provider configure the most frequent codes to appear in the Multiple Code Selection screen, which are configured in the Settings / Clinical / Frequent Codes screen.
