There are several options for creating a new patient record.
Option #1
- Select the Patients button in the Main Menu.
- The following screen will be displayed:
- Click the Add button located at the top of the screen.
- Enter the patient's demographic information. All required fields are marked with a red asterisk. (*).
- The required fields are:
- Name
- Gender
- Race
- Ethnicity
- Civil Status
- DOB
- Birth Sex

- The required fields are:
- After registering the patient's demographic information, the user can upload a profile or ID picture in the Pictures section, which is located on the right side of the screen.
- To verify a patient's eligibility, click on the Insurance tab located on the right side of the screen.
- The section will be displayed to register the patient's insurance information.
- After registering the patient's insurance information, click the Eligibility button.
- After registering all the patient's information, click the Save button to generate a patient record number.
Option #2
- Click on the QuickLinks text, located at the top right of the screen, in the main menu.
- Choose the Patients option.
- The following screen will be displayed:
- Click the Add button located at the top of the screen.
- Enter the patient's demographic information. All required fields are marked with a red asterisk. (*).
- The required fields are:
- Name
- Gender
- Race
- Ethnicity
- Civil Status
- DOB
- Birth Sex

- The required fields are:
- After registering the patient's demographic information, the user can upload a profile or ID picture in the Pictures section, which is located on the right side of the screen.

Option #3
- Navigate to the Appointments screen.
- Choose the new patient from the calendar.
- After selection, the user has two options for record creation. They can click on the Patient Details button, which is situated on the right side of the screen.
- Alternatively, press the Information button located in front of the patient's name and choose the Create Rec option.
- In either scenario, the subsequent screen will appear:
- Choose the patient's gender and date of birth. Next, check the box labeled Go to patient details.
Click on the Save button.
Subsequently, the screen below will be displayed:
- Enter the patient's demographic information. All required fields are marked with a red asterisk. (*).

- Name

- Race
- Ethnicity

- DOB
- Birth Sex

Once the patients demographic information has been registered, user can add a profile or id picture, in the Pictures section, located at the right side of screen.
- The required fields are:
If patient has a medial plan, click on the Insurance link, located at the right side of screen. The section will be displayed, in order to register the patients insurance information.
- To check patients eligibility, press the Insurance tab, located at the right side of screen.
- The following screen will be displayed, in order to add the patient's insurance information.
- Once the patient's insurance information is registered, click on the Eligibility button.
- Once all patient's information is registered, click on the Save button. This will create a patient record number.
