• Go to the Advanced Progress Note screen.
  • Click on the Documents button located at the Side Bar section. Or click on theSearch button located at the left side of screen and then select the Documents button.


  • The following screen will be displayed: 


  • Click on the Consent Forms button located at the upper side of screen. 


  • All the registered consent forms in system will be displayed in screen.
    • Note: To add a consent form Template, go to Settings / Office / Template, select the main category in GENERAL and the category in Consent Form.
  • Select from the left chart the consent form.
  • Click on the Select button.  

       

  • The following screen will be displayed:

       

  • Click on the Replace keywords and close button. 
    • IMPORTANT NOTE: If document has a sign registered [Keyword], the Signature Pad must be installed.

  • This Template has a patient sign registered Keyword. The following screen will be displayed to take the patient's signature.


  • If patient signed correctly, click on the "OK button. 
  • The provider providers sign in this example will be automatically displayed. 
    • Note: In order to the provider signature be automatically displayed it must be registered in system in the Default Provider" field in the "Patients" screen. It must has to be filled with the provider name.
  • When system finishes to replace the Keywords, document will be displayed in screen.

   

  • The following confirmation message will be displayed: 

          

  • Click on the Yes button to store consent form. 
  • Click on the No button to cancel action. 
  • Another message will be displayed:

         

  • Click on the Yes to indicate that document has an advanced directive.
  • Document will be displayed on screen.
