EThere are several options for documenting patient questionnaires.


Option #1

  • Go to the Advanced Progress Note screen.
  • Click on the Evaluation Form History link, located in the Patient Dashboard section.


  • The following screen will be displayed:


  • If you wish to add a frequent questionnaire, select from the Frequent Questionnaire section.


  • The questionnaire will be displayed in the Questionnaire section.


  • Once you have recorded the information, select the result, according to the answers of the questionnaire.


  • If you wish to include the answers from the questionnaire in the report, select the Include answers in report checkbox, located at the bottom left of the screen.


  • Once you have finished recording all the information in the questionnaire, press the Save button to save the questionnaire.
  • To print the patient questionnaire, press the Print button located at the top of the screen.


  • Automatically, the questionnaire will be displayed, to be printed.


Option #2

  • Go to the Advanced Progress Note screen.
  • Press the Add button, located in the Evaluation Form History section of the Patient Dashboard.


  • The following screen will be displayed:


  • If you wish to add a frequent questionnaire, select from the Frequent Questionnaire section.


  • The questionnaire will be displayed in the Questionnaire section.


  • Once you have recorded the information, select the result, according to the answers of the questionnaire.


  • If you wish to include the answers from the questionnaire in the report, select the Include answers in report checkbox, located at the bottom left of the screen.


  • Once you have finished recording all the information in the questionnaire, press the Save button to save the questionnaire.
  • To print the patient questionnaire, press the Print button located at the top of the screen.


  • Automatically, the questionnaire will be displayed, to be printed.



Option #3
