There are several options for deleting patient questionnaires.


Option #1

  • Go to the Advanced Progress Note screen.
  • Click on the Evaluation Form History link, located in the Patient Dashboard section.


  • The following screen will be displayed:


  • Press the History button, located at the top of the screen. 


  • The following screen will be displayed.


  • Select the questionnaire, which you want to delete.


  • Once selected, press the right mouse button.
  • The Delete option will be displayed.


  • When pressed, a confirmation message is displayed.


  • Pressing the Yes button displays a second confirmation message, since this action cannot be reversed.


  • Pressing the Yes button will automatically delete the questionnaire.


Option #2

  • Go to the Advanced Progress Note screen.
  • Press the Add button, located in the Evaluation Form History section of the Patient Dashboard.


  • The following screen will be displayed:


  • Press the History button, located at the top of the screen. 


  • The following screen will be displayed.


  • Select the questionnaire, which you want to delete.


  • Once selected, press the right mouse button.
  • The Delete option will be displayed.


  • When pressed, a confirmation message is displayed.


  • Pressing the Yes button displays a second confirmation message, since this action cannot be reversed.


  • Pressing the Yes button will automatically delete the questionnaire.


Option #3

  • Go to the Advanced Progress Note screen.
  • Click on the Evaluation Form History link, located in the Patient Dashboard section.


  • The following screen will be displayed:


  • Press the History button, located at the top of the screen. 


  • The following screen will be displayed.


  • Select the questionnaire, which you want to delete.


  • Once selected, press the right mouse button.
  • The Delete option will be displayed.


  • When pressed, a confirmation message is displayed.


  • Pressing the Yes button displays a second confirmation message, since this action cannot be reversed.


  • Pressing the Yes button will automatically delete the questionnaire.
