Displays the 1500 form (Health Insurance Claim Form) (02-12)



Access

Select claims through a Check mark in the Billing screen located in the left side of the claim number. Press the Print button  once claim is selected and then select the CMS 1500 option. When the 1500 form is printed the claim status will change to Printed. More than one claim can be printed at the same time.
