Option #1
- Modern Main Menu: Click on the "Eligibility" button.
- Classic Main Menu: Click on the "Eligibility" icon, situated in the upper right corner of the screen.
Option #2
- QuickLinks: Click on the "QuickLinks" text at the top of the screen, then choose the "Eligibility" option.
- The screen that will be displayed is as follows:
Perform the following steps:
- Document the patient's first and last name.
- Document the patient's date of birth.
- Select the gender of the patient.
- Record the patient's address. The city field will be filled, when you register your zip code.
- Record the patient's medical plan.
- Once the information is registered, press the "Services" button, located at the top of the screen. Or press the "Refresh" button, located in the center of the screen.
