Option #1

  • Modern Main Menu: Click on the "Eligibility" button. 


  • Classic Main Menu: Click on the "Eligibility" icon, situated in the upper right corner of the screen.


Option #2

  • QuickLinks: Click on the "QuickLinks" text at the top of the screen, then choose the "Eligibility" option.


  • The screen that will be displayed is as follows:


Perform the following steps: 

  • Document the patient's first and last name.
  • Document the patient's date of birth.
  • Select the gender of the patient.
  • Record the patient's address. The city field will be filled, when you register your zip code.
  • Record the patient's medical plan.
  • Once the information is registered, press the "Services" button, located at the top of the screen. Or press the "Refresh" button, located in the center of the screen.
