Description

Screen used to document and store the patient's vital signs.


Access 

  • Through the Side Bar section: To access the Vital Signs Main button on the Progress Note screen, click on the Side Bar. If the Vital Signs Main button is not visible, click on the Settings button on the Progress Note toolbar, then choose Side Bar Settings. From the available items on the right, select Vital Signs Main and drag it to the items window in the lower left corner of any Side Bar category. Click Save to apply the changes. The Side Bar will update, and the button will be displayed under the chosen category.
  • Through the Search section: Click on the Search button located on the left side of the screen, then select the 'Vital Signs Main' button.
  • Through the Patient Dashboard section: To access the Vital Signs, click on the corresponding link in the Patient Dashboard, or to view the full page, click on the Add button and choose the Open Full Page option.

Buttons

  • Settings: It will display the Settings screen of the vital signs.
  • Print: Prints report of vitals read.
  • Graphics: It will display a screen where you will be able to view graphs on the following areas:
    • Weight
    • Height
    • Breath
    • Pulse
    • Temperature
    • BP
    • Pain
    • BMI
    • Weight and Height
  • Pediatric Chart: It will display, where the following graphs can be viewed separately: Length for Age, Weight for Age and BMI for Age.
  • Add: Allows adding vital signs.
  • Edit: Allows editing vital signs.
  • Delete: Allows the patient's vital signs to be deleted.
  • Cancel: Cancels any operation you wanted to save.
  • Save: Saves the changes made on screen.
  • Previous: Closes the screen.


Reading Details

  • English or Metric: Select the vital signs reading format of your choice.
  • Reading Date/Time: Date results were read to patient.
  • Height: Height of the patient at the time of recording the data.
  • Weight: Weight of the patient at the time of data entry. In the case of pediatric patients, the oz field will be displayed.
  • BMI: Body Mass Index. Body Mass Index. This information will be displayed automatically, once the patient's height and weight are recorded.
  • Head Circumference: Head circumference.
  • Glucose Test: Glucose test taken from the patient, at the time of data recording.
  • Sp02: Measurement of percentage of molecules, bound to oxygen.
  • ¡02: 
  • GFR (Glomerular Filtration Rate): Recording of glomerular filtration rate result, which determines the proper function of the kidneys.
  • R/Rate: Respiratory frequency of the patient at the time of recording the data.
  • Pulse: Arterial pulse rate of the patient at the time of data recording.
  • Temp: Temperature of the patient at the time of recording the data.
  • Systolic: Result that the patient had at the time of recording the data.
  • Diastolic: Result that the patient had at the time of recording the data.
  • Pain: Pain that the patient had at the time of recording the data.
  • Counseling for Nutrition: Check option, if nutrition counseling was given to the patient.
  • Counseling for Physical Activity: Check option, if physical activity counseling was given to the patient.
  • Denied Readings: By selecting, you are indicating that the patient does not want the vital signs to be displayed.
  • Notes: User will be able to write a note, related to the taking of vital signs.
