Several options are available to reconcile the patient's medications.

Before

  • Patient must give consent: To proceed with medication reconciliation, one must first verify the pharmacy's eligibility, ensuring it is active. Additionally, it is necessary to select the 'Consent for Health Information Exchange' option found in the Patient Details screen. (This service is unlimited; however, not all health insurances provide it.)

After


Option #1

  • Go to the Advanced Progress Note screen.
  • Click on the Medication List link, located in the Patient Dashboard section.


  • The following screen will be displayed:


  • Press the More button located at the top of the screen and select the Reconcile option to perform the electronic drug reconciliation.


  • The Patient Pharmacy Network History screen will be displayed.


  • Press the Agree button.
  • Once pressed, the downloading process will begin.


  • Select in the Unreconciled column, all those medications that you wish to add to the patient's medication history. 


  • If the drug is saved, but Uncoded/outdate is displayed in the NDC column, right-click on the drug and select the corresponding dose.


Option #2

  • Go to the Advanced Progress Note screen.
  • Press the More button, located next to the Medications List text and select the Open Full Page option.


  • Or, press the right mouse button and select the Open Full Page option.


  • The following screen will be displayed:


  • Press the More button located at the top of the screen and select the Reconcile option to perform the electronic drug reconciliation.


  • The Patient Pharmacy Network History screen will be displayed.


  • Press the Agree button.
  • Once pressed, the downloading process will begin.


  • Select in the Unreconciled column, all those medications that you wish to add to the patient's medication history. 


  • If the drug is saved, but Uncoded/outdate is displayed in the NDC column, right-click on the drug and select the corresponding dose.


Option #3

  • Go to the Advanced Progress Note screen.
  • Press the Medication History button, located in the Side Bar section. 


  • Or, press the Search button located at the top of the screen and select the Medication History button.
  • The following screen will be displayed:


  • Press the More button located at the top of the screen and select the Reconcile option to perform the electronic drug reconciliation.


  • The Patient Pharmacy Network History screen will be displayed.


  • Press the Agree button.
  • Once pressed, the downloading process will begin.


  • Select in the Unreconciled column, all those medications that you wish to add to the patient's medication history. 


  • If the drug is saved, but Uncoded/outdate is displayed in the NDC column, right-click on the drug and select the corresponding dose.
