Denominator

Patients seen during the evaluation period with at least one closed progress note. 


Numerator

Patients with a Problem List registered during the evaluation or indicated with No known problems or has a specific diagnosis reviewed. 

 

What to do in EHRez?

Patient must have at least one diagnosis registered in the Problem List screen. If patient does not have a diagnosis to be registered press the No Known Problems button. If patient has a registered diagnosis select it and press the Selected Problem List Reviewed button. The numerator goal is  80%.



Steps to upload measure

There are three options. Select the best option according to the patients situation. 

 

Option #1: Record patient diagnosis.


Option #2: Indicate that patient has no known problems.
