Percentage of children 2 years of age who had 4 diphteria, tetanus and acelluar pertussis (DTaP); 3 polio (IPV), and one measles, mumps and rubella (MMR), three H influenza type B (Hib), 3 hepatitis B (Hep B); one of chickenpox (VZV); four pneumococcal conjugate (PCV); one hepatitis A (Hep A); two or three rotavirus (RV); and two of influenza (flu) vaccines by their second birthday.


Denominator

 2 years old patients with at least one progress note closed during the measure period.

 

Numerator

Patients in denominator that received the recommended vaccines and were documented.

 

What to do in EHRez?

In the Immunizations screen record the 2 years old patients, 4 vaccines (DTaP), 3 (IPV), 1 (MMR), 3 (HiB), 3 (Hep B) 1 (VZV), 4 (PCV), 1 (Hep A), 2 o 3 (RV) and 2 (Flu) with the corresponding dates.
