Percentage of children from 6 to 12 years old that was prescribed with a medication for the attention deficit/ hyperactivity (ADHD) that has an suitable health follow up. There will be two percentage reported:

 

 

CMS136v4-1: Children 6-12 years of age who were dispensed an ADHD medication during the Intake Period and who had a visit during the measurement period.


Denominator 

Patients of 6 to 12 years old with an ADHD** medication prescription and has a registered progress note during the report period.  

             

Numerator

Patients with at least one registered visit and one authorized prescription between the 30 days after the IPSD (Index Prescription Star Date). 

 

What to do in EHRez?

Patients between the ages of 6 to 12 years with a prescribed medication for ADHD according to the measure recommendation**. 

            

Example of measures to be uploaded.

  • # 1: 9 year old patient with a Major Depression Disorder-Active diagnosis. ICD9:296.20 [ICD10:F32.0], with a Amphetamine-Dextroamphetamine 7.5 mg [RxNorm = 1009145] prescription and added a 99202 code when closing the encounter.
  • # 2: 7 year old patient with a Dextroamphetamine Sulfate [884684] prescription and added a 99202 when closing the encounter. 

 

CMS136v4-2: Children 6-12 years of age who were dispensed an ADHD medication during the Intake Period and who remained on the medication for at least 210 days out of the 300 days following the IPSD, and.

           

Denominator

Patients of 6 to 12 that has an  ADHD** medication prescription during encounter and has been with medication during at least 210 days out of the 300 following days of medication and has at least a visit during the measure period. 

             

Numerator

Patients belonging to the denominator and has a initial visit and at least two follow up visits for the continuity and maintenance plan.
