Denominator
18 to 85 years old patients with Essential Hypertension diagnosed during the first 6 months of the measurement period or before.
- Important Note: The Essential Hypertension diagnosis that apply are only the following: ICD9: 401.0, 401.1, 401.9, ICD10: I10 **
Numerator
Patients belonging to denominator,that in recent visit had there blood pressure controlled. (Systolic < 140mmHg /Diastolic < 90 mmHg)
What to do in EHRez?
Patients between the ages of 18 to 85 years with a hypertension diagnosis**, take vital signs and by this is demonstrated that blood pressure is controlled.
Example to upload measure.
Example: 72 years patient with a hypertension diagnosis in the first encounter (ICD9: 428.0) and had the vital signs taken with 150/60 ,and on the second encounter had registered.
