Percentage of patients aged 18 years and older who were screened for tobacco use one or more times within 24 months and who received cessation counseling intervention if identified as a tobacco user.

Denominator

Patients of 18 years or older that have more than one encounter and have the Smoking Status field registered during the report period.

Numerator

Patients registered in the denominator that has the Cessation Consultation option selected during the report period.

What to do in EHRez?

Register all patients the Smoking Status in the Smoking Status screen. And all smoker patients to be orientated to smoking cessation and mark the Cessation Consultation option on the same screen. Patient must have registered more than one encounter in record during the measure period.

Examples to upload measures.

# 1:
49 year old patient
- 1st encounter:In his first encounter on 5/11/2014 a Smoking Status = Light tobacco smoker was registered.
- 2nd encounter: On his second encounter on 8/11/2014, was orientated to quick smoking and was marked with a Check mark the Cessation Consultation option.
# 2: 77 years old patient with a previous encounter. During his second encounter patient was registered in the Smoking Status like a Never smoker patient.
