Percentage of patients aged 12 years and older screened for clinical depression on the date of the encounter using an age appropriate standardized depression screening tool AND if positive, a follow-up plan is documented on the date of the positive screen.
Denominator
12 years or more patients that have at least one closed progress note during the period report.
Numerator
Patients with a depression clinical evaluation (Questionnaire) in visit using the appropriate tools for patients age.
Exclusions
Patients with a previous depression and/or bipolarity diagnosis.
What to do in EHRez?
Patients presenting certain symptoms of depression, make a depression evaluation, in the Evaluation Form screen and select any of the following evaluations according if patient is an adult or a teenage:
- Beck's Depression Inventory
- Geriatric Depression Scale (GDS)
- Hamilton Rating Scale for Depression
- Patient Health Questionnaire (PHQ-9) - [Adolescent]
- Patient Health Questionnaire (PHQ-9) - [Adult]
- PROMIS - Depressive Symptoms - [Parent]
- PROMIS - Depressive Symptoms - [Pediatric]
- PROMIS - Emotional Distress - Depression - [Adult]
- Zung Depression Scale

- # 1:15 year old patient without a previous depression diagnosis and had (ICD(: 296.21) (ICD10: F32.0) with a PHQ-9 Adolescent questionnaire preformed in the Evaluation Form screen. With a Negative result.
- # 2: 56 yeras olda patient with a 296.21 (F32.0) diagnosis and with a PHQ-9 Adult questionnaire performed in the Evaluation Form screen. With a Positive result.
