All patientes 18 through 64 years on the date of the encounter
Denominator
Patients between 18 to 64 years old at the encounter date and has at least one closed progress note.
Numerator
Patients belonging to denominator which weight and height were documented. (Note:The BMI is automatically calculated when weight and height is registered.) and maintained a >=18.5 kg/m2 y < 25 kg/m2 BMI during the previous 6 months. Take into consideration when BMI is out of the normal parameters a follow up is documented.
Exclusions
Patients in pregnancy.
What to do in EHRez?
Register the Height and Weight to all patients between the ages of 18 to 64 years old (The BMI will be automatically calculated). If BMI is within the levels mentioned above, the measure will upload.
All patients 65 years of age and older on the date of the encounter
Denominator
Patients of 65 years old at the encounter date and has at least one closed progress note. .
Numerator
Patients belonging to denominator which weight and height were documented (Note: The BMI is automatically calculated when weight and height is registered) during the previous 6 months. Take into consideration when BMI is out of the normal parameters a follow up is documented.
Exclusions
Patients in pregnancy.
What to do in EHRez?
Register the Height and Weigh to all patients between the ages of of 65 years or older (The BMI will be automatically calculated). If BMI is within the levels mentioned above, the measure will upload.
