Description

The screen displays the patient's medication history for up to two years and is capable of performing medication reconciliation.

Access 


Through the Side Bar section: To access the Medication History, click on its button in the Side Bar of the Progress Note screen. If you need to reconcile, click the More button on the screen's toolbar and choose the Reconcile option. If the Medication History button is missing, go to the Settings on the Progress Note screen's toolbar, select Side Bar Settings, and drag the Medication History from the available items on the right into the items window on the lower left of any Side Bar category. Save your changes, and the Side Bar will update to display the button in the chosen category.

Through the Search section: Click on the Search button located on the left side of the screen, then select the Medication History button. Next, click on the More button found on the screen's toolbar and choose the Reconcile option.

Through the Patient Dashboard section: To access the Medication List, click on the corresponding link in the Patient Dashboard or select the 'Add' button to open the full page option. Next, click on 'More' in the screen toolbar, choose the 'Admin' button, and then opt for the 'Reconcile' feature.


Configuration

To use this functionality, make sure to review the following:

  • Pharmacy Eligibility: Pharmacy coverage eligibility must be performed, and it must return at least one (1) active coverage.
  • Patient consent is documented: Patient consent is documented in the Patient Details screen. Check the Consent for Health Information Exchange option at the bottom of the screen. This consent allows EHRez to post clinical health information to Health Information Exchanges and return medication history.  

Sections

  • Disclaimer: User disclaimer. Accept to proceed with medication history download.

Buttons

  • Reconcile: Press to add selected medication to the patient's medication list.
  • Previous: Returns to previous screen.

Column [Patient Medications]: Displays the active medication in the patient's medication list.

Column [Unreconciled]: Displays medication present in the network history list and not on the patient's medication list.

Column [Pharmacy Network History]: Displays medication in the network history (up to 2 years).


Error Messages


Error
Description
Prescriber Phone is missing.Problem: The provider's phone number field has not been properly entered.
Solution: go to Settings / General /Users. Select the appropriate user and complete the phone number entry.
Consent Not GivenProblem: Patient record does not reflect consent for Health information Exchange.
Solution: Go to Patient Details, edit the patient record and authorize the Consent for Health Information Exchange option.
